If at least one of the names on the contract does not match the Utility account holder: The Utility account holder will need to add or switch names to match the name on the contract (all products). This is required for the Interconnection process approval by NV Energy
HOA Questionnaire	E-Signature	System Owner	Palmetto is not responsible for HOA recourse against clients if they answer "no" on this form.
